Evidence of active learning - is an OTJT log sufficient?

The ESFA requirement is that for apprenticeships there must be active learning taking place and there must be evidence for this at least every 4 weeks.
If learners are completing an off the job training log, is their entry written into the log sufficient, or does the entry need supporting evidence in order to qualify as evidence of active learning?
