Audit recommendation: evidence between prime and subcontractor

During a recent audit we were told that our enrolment form (or similar document) must clearly outline the roles and responsibilities between the main provider and subcontractor. They were looking for something that states what part of the course the subcontractor has responsibility for and what the main provider does - this was required for AEB as well as devolved funding.
We are covered on apps delivery but need to put something together for AEB/16-19/FCFJ delivery.
If anyone has a template they'd be willing to share with me, I'd be grateful.
