16-19 Change of Learning Plan

Good morning
Can anyone please clarify if we need to amend the original plan/paperwork if a learner falls behind with their aims on 16-19 SP?
For example;
A learner who has a core aim, maths and English opens in August and is due to finish those aim in November but due to poor attendance they have gone past those aim PED.
The remaining aims of enrichment and work related activity should of been opened in December against the original plan however these will now be open in February once the original core and FS aims are achieved.
Would we need to show a revision of the original plan with a learner signature to explain why the remaining EEP aims have been pushed back and now don't match up with the original plan?
We keep very accurate attendance records and only open aims in the ILR if we can evidence if a learner has attended an aim. We also document progress at aim level on weekly review documents as well.
