This situation comes up sometimes and I'm trying to think about what action we need to take in terms in documentation.
We deliver a degree apprenticeship made up of individual modules, each of which ends with an on-programme assessment. From our point of view, if they fail the assessment then they haven't earned the credits, and so will be required to either retake the assessment, or else resit the module. Retaking requires them to undertake this work alongside their continuing study on other modules. Resitting pauses the other modules until they have passed.
Our Training Plan allocates timing and OTJ hours on the assumption that each module will be passed first time, which means that once a fail occurs they will start to depart from the plan, by additional hours and/or revised timing. Is an auditor likely to query whether resit hours count as new learning (or OTJ in general)? Are we required to amend the Training Plan?
