Failed Maths but passed EPA

Good morning, I have a learner that I put forward for EPA before gaining their Maths under the special measures due to Covid. EPA was completed with a distinction in May 2021. 

We have had so many issues with the Math exam. The first exam was sat in January but there was some kind of mix up with the papers and the awarding body refused to accept them. We put the learner forward for the EPA as it was felt by the Maths provider that she would pass. The learner sat the Maths exam again in June,  but unfortunately failed it. 

The learner now has significant mental health issues and is citing the Maths exam as the sole source and has asked for an extension to October/November before sitting it again. 

Im a bit confused with the rules now but believe that this isnt possible? 

Any help/advice appreciated
