Not sure a form will do for the evidence of employment. But at least getting the payslip would help with the PAYE evidence.
Will the ESFA be updating the Apprenticeship Service so that you can see the employers PAYE - or is there a way you can see it at the moment?
"The employment is for a period which is long enough for the apprentice to complete the apprenticeship successfully" - makes me think of someone who could not sign up for an apprenticeship at the moment. How you can evidence this is beyond me - can we evidence there will be no bankruptcy/epidemic/war/etc. Should it be: "They are employed on a permanent contract or a temporary contract that covers the apprentice until the planned end of the apprenticeship."

Employment arrangements (see paragraphs P26 to P27) P324 The evidence pack must include: P324.1 Evidence of employment (e.g. contract of employment, payslip, HMRC record) and that this employment is for a period which is long enough for the apprentice to complete the apprenticeship successfully, including the end-point assessment. Employed status will be cross-checked with HMRC
Do we have to collect a physical document, or could we ask questions on a signed document?
