We are a provider and would like to find out how other providers determine which is the primary LLDD and health problem for an apprentice who has more than one so it can be accurately reported in the ILR.

Just pick one, it's not like it makes any difference? If in doubt, I tend to pick the lowest value.
Yep, we just ask them. We word it as "If you select more than one, please indicate the one you consider to have the most significant impact on your education". How they indicate that depends on how they applied/enrolled (online, paper, face to face).
