Hi I have an apprentice who has been on a break in learning 13/09/2022 on maternity leave. She has now decided not to return. What date do I make the leaver date? Today as, this is when the employer and apprentice have let us know? Today is the date the apprentice DAS record has ben completed.

No, you leave the original Actual End Date in there, and just simply from Completion Status 6 to 3. The DAS record should be stopped back to Sep 2022, but it's not really a problem if the stop date is this month (unless she's trying to start another apprenticeship).
