Hi Everyone

Is there a template for this anywhere or a definitive list of what this needs to include? So far I have the following:

Learner details

Reason for leaving

Collection of co-investment payments

Incentive payments made to employers

Employer stopped funding in app service

You don't, as such, need a withdrawal form, just all the pieces of evidence you've listed, which probably "naturally occur" in other processes you have (so, eg, evidence of the App being stopped on AS would be demonstrated by you logging on to AS and showing the auditor that it had been stopped)

With the AS, what date do employers put in there? Is it LDOL or something different?

If you use an e-portfolio system most have forms embedded in them which could be used for this purpose. For example we use Smart Assessor and use the exit form within Smart when a learner goes on a break or withdrawals. Not saying its compulsory, just if you use something similar the form may already be there.

As far as I'm aware, yes. Suspect it's only the month, not the day though (and make sure you both agree first! If they put a date earlier than you put in the ILR you'll lose money, also stop date is pretty much un-updateable!)
