Paragraph 389 says gateway to completion is "when they have satisfied all the requirements set out in the assessment plan", which would include OTJ hours. I do think they should list minimum hours explicitly in there.
Having said that, I'd personally say yes to the second one, as long as it's new and related to a KSB.
I think whilst still on programme, it makes sense that you could still be doing new stuff even post assessment, but once you're right at the end and ready to claim, you should be fully ready to pass with no extra delivery, so any optional extra delivery I wouldn't count. I think of it like, you can still pass a degree even though you didn't know all the content. There are still things you could learn afterwards that are absolutely relevant to the degree, but if you already sat the final exams, any learning after that isn't part of the degree. If you're in year 2 but still learning stuff about a modules you already passed in year 1, that's still part of the degree as you're still learning and not ready for the final exams yet. That's just my interpretation though.

Afternoon,
I have a couple of queries in relation to the new assessment reforms, I’m hoping somebody is able to help.
Are learners expected to continue with OTJ and monthly ‘active learning’ after they have reached ‘gateway to completion’? Would learners need to have met the minimum OTJ at the point they reach the ‘gateway to completion’ as paragraph 389 doesn’t list it?
Also, if learners have sat an on programme exam that covers specific Knowledge and Skills topics, are learners able to claim OTJ for related for those K&S after starting the assessment as there could still be new knowledge/skills even though they’ve passed an exam?
