If we need to extend an apprentice's programme duration part-way through their programme, do we also need to increase the OTJ hours? I know we don't amend anything on the ILR which is confusing me!

You do not say why you need to extend the duration if it is due to a reduction of employment hours you should reflect this change on the commitment paperwork only, the OTJ hours remain as originally entered on the ILR and should not significantly change.
