Nope, alongside internal reports I have also tried using the Apps Indicative Earnings Report to apply the two sets of conditions - still no match.

Yes, however at R03 we noticed too much had been collected and this was refunded to the employer and corrected on the ILR, however learner completed in R01. Would this cause the achievement payment to be held back?
